For AV v.3.3 and SDE v9.1, install Database Access 2.1g upgrade
Add to end of c:\windows\system32\drivers\etc\services file:
sde_mysde 5151/tcp #sde service on \\mysde sde_yoursde 5151/tcp #sde service on yoursde.example.com
In Windows XP, if firewall protection is on, you need to add port 5151 to open list. For performance reasons -- when inside a protected network -- it's better to keep the firewall turned off.

Press the [connect] button to test. If successful, [next] and then choose your themes. You can just re-use the existing connection for the duration of the session (when you start AV anew you'll had to do this again).
I was pleasantly suprised to discover that it "just works" and that it is noticably faster than using coverages via ArcLibrarian.
